Clinical formulation
Subacute high (C4 or above) cervical myelopathy, likely due to a compressive lesion affecting the anterior cord bilaterally. A disc prolapse is the most likely cause, alternatively a tumour. Pattern not typical of non-compressive causes - but this should be be considered if imaging unrevealing.
