Clinical formulation
Sudden-onset vertical and horizontal binocular diplopia with skew deviation, right INO and central pattern nystagmus, in a man with multiple vascular risk factors - most likely due to a lacunar stroke in the mid- or upper-pons affecting the right dorsal tegmentum where the MLF ascends.
